Surveillance System to Observe Seroconversion to SARS COV2 in Human (SYNTHESIS): Combining Hospital-Based and Population-Based Longitudinal Study in Sleman HDSS (Health and Demographic Surveillance System)

A new Systemic Acute Respiratory Syndrome (SARS) emerged in December 2019 in Wuhan, China. Later, this syndrome was called Coronavirus Disease 2019 (COVID-19), an infectious disease caused by SARS-CoV-2 infections that brought cataclysm both in population and health care settings. SARS-CoV-2 showed different features than the 2002 SARS Pandemic, with slower clinical manifestation, easier transmission, and yet to confirm related immunological aspects, transmission rate, or risk factors.

The SYNTHESIS Project is hospital and population-based longitudinal study aimed to understand the natural course related to SARS-CoV-2 infection in Indonesia, from population to clinical perspective. These multi-year international collaborative efforts started in 2021 to 2023 between Universitas Gadjah Mada, academic hospitals, government bodies, and Perdana University Center for Research Excellence Malaysia were funded by the Indonesia Endowment Fund for Education (LPDP) and Indonesia Science Fund (DIPI) Ministry of Finance through RISPRO International Collaboration Scheme. This project was divided into three major objectives:

  • Study 1 – Observation of IgM-IgG Titer to SARS-CoV-2 Seroconversion
  • Study 2 – Risk and Protective factors of SARS-CoV-2 infection in Population
  • Study 3 – Open Data Practice Initiative and Development

Study 1 – Observation of IgM-IgG Titer to SARS-CoV-2 Seroconversion


There is an urgent need for assays that measure the SARS-CoV-2 antibody response and determine seroconversion in the population in Indonesia. The observation was conducted in UGM Academic Hospital in Sleman, Yogyakarta for 18 months. Data collected from the observation process include biosample, demographic data, clinical characteristics, comorbidity, and clinical parameters that correlate with the seroconversion. This objective will identify when and how often a serological test of SARS-CoV-2 should be done and how severe the patient’s condition will further determine more precise clinical management and future development of COVID-19 vaccine policy in Indonesia.

Study 2 – Risk and Protective factors of SARS-CoV-2 infection in Population


In the general population, incidence and risk factors for COVID-19 are needed to plan an appropriate disease prevention model. The SARS CoV-2 is spreading more rapidly and burdensome to public health than the SARS CoV-1 because the virus is more contagious and has higher asymptomatic cases. Therefore, how the virus interacts with intrinsic and extrinsic factors specific to Indonesia is needed to be explored. Further, these will shape the determination of risk and protective factors for SARS-CoV-2 infection in Sleman District by comparing cases and controls groups and investigating the factors associated with SARS-CoV-2 seroconversion in the Sleman population.

Study 3 – Open Data Practice Initiative and Development


To develop a database management system to link and manage data obtained from activities related to the previous study objectives as well as data from UGM-HDSS Sleman, hospitals (UGM Academic Hospitals), and government bodies (Department of Population and Civil Registration, Department of Health, Department of Information and Communication of Sleman Regency). Proposal formulation for an open data regulatory framework in Indonesia with diaspora partners and stakeholders, developing data management and data sharing models that apply ethical, legal, and social principles.
