Terdapat dua kode modul anggota rumah tangga yang digunakan dalam kuesioner HDSS Sleman yaitu ART dan PART.

Modul ART berisi pertanyaan mengenai informasi demografi dasar responden yang belum pernah tercatat oleh HDSS Sleman, dengan kata lain anggota rumah tangga baru.
Modul PART untuk responden lama, perubahan data demografis yang ditanyakan antara lain adalah perubahan status kepala keluarga, keberadaan (ada, meninggal, atau pindah), status perkawinan, pendidikan, pekerjaan, dan kepemilikan asuransi kesehatan.
